Registration for the upcoming 2008-2009 Travel Soccer Season will be June 4th, from 6:00PM to 9:00PM at OB Gates Elementary School.
As always we are looking for volunteers to help at Registration. Please contact Ann at office@chesterfieldunited.com if you are willing and available.
All players & parents must attend registration night. This year we will be collecting all paperwork, and trying on uniforms for ordering. Cost of the uniform package is $125.00 (We will be accepting credit card payments and checks) All forms required for registration can be printed off this site (Links to the left). There will be a photographer and a notary onsite if you wish to purchase the 1"x1" head shot or notary services for the medical release form. Additionally, the team warm-up, backpack, hoodie, and practice jersey will be available for sizing.
The CUFC Player & Parent Commitment Guidelines were handed out at tryouts and are posted on this site. Please be sure you have read them in their entirety.
ACADEMY PLAYERS U9-U12 and U11 Elite supply the following:
- 2 pictures (head shots) 1" x 1" (can be purchased on site)
- Original birth certificate and 1 copy. (Not needed for 07-08 CUSC ADSL players)
- US Club Soccer Authorization and Medical Release Form (notarized)
- CUFC Player & Parent Commitment Signature Page
ELITE PLAYERS U12-U18 and all CLASSIC PLAYERS supply the following:
- 2 pictures (head shots) 1" x 1" (can be purchased on site)
- Proof of Birth-original birth certificate, passport, driver’s or walker’s license (Not needed for 07-08 CUSC VSLI players)
- VYSA Medical Release Form (notarized)
- VYSA International Clearance Waiver (Needed for players who have lived outside of the US)
